Pronto hará 20 años, unos especialistas del St. Luke 's Hospital de Nueva York publicaron un artículo en el que sugerían que la actividad física en la anorexia nerviosa no era simplemente consecuencia de la decisión de incrementar el gasto calórico (Kron et al, 1978). En un grupo de 33 pacientes hospitalizadas, 25 presentaban "hiperactividad" durante su enfermedad; de estas 25, 21 parecían haber incurrido en una actividad física desusada antes de iniciar la restricción alimentaria y la pérdida de peso; en un estudio de seguimiento se entrevistaron 13 pacientes: 10 proseguían con un exceso de actividad física a pesar de que en su mayoría habían recuperado peso significativamente y algunas habían visto desaparecer la amenorrea.
