Resumen

La aparición o el empeoramiento de los tics durante el tratamiento con estimulantes es relativamente freruente en la clínica, y está sujeta a controversia en sus causas y en la responsabilidad atribuible a los estimulantes. Éstos podrían 1) favorecer la aparición de tics, 2) adelantar su presentación, 3) incrementar los ya existentes o 4) coincidir casualmente con la aparición o el incremento de tics. En un niño de ocho años, con antecedentes personales y familiares de tics, se describen el empeoramiento de los tics ya existentes, coincidente con la administración de metilfenidato, y su casi desaparición tras la retirada del fármaco a los tres meses de iniciado el tratamiento. El empeoramiento tras el uso de estimulantes puede sugerir la existencia de factores individuales como la ictericia neonatal que, ligados o no al factor genético de estos trastornos, en caso de identificarse contribuirían a la prevención de un riesgo que mientras no sea aclarado exige diversas medidas de precaución, o fármacos alternativos, antes de la administración_ de estimulantes.
