Abstract

Introduction: There is increasing evidence on the validity of the diagnosis of pediatric bipolar disorder. There are multiple factors that contribute to the disease being underdiagnosed and easily confused with other disorders with pathological externalizing behaviors, such as attention deficit hyperactivity disorder.

Clinical case: The case of a child with suspected childhood bipolar disorder is exposed. He has attended intensive follow-up from 3 to 5 years, in order to observe their behavior and carry out an adequate differential diagnosis.

Conclusions: The diagnosis of pediatric bipolar disorder should be considered in children with severe symptoms of hyperactivity, distractibility, impulsivity and behavior alteration, especially in those with a family history of bipolar disorder, flashy mood fluctuations, episodes of aggressiveness severe and sleep disturbance.
