El trastorno por déficit de atención / hiperactividad (TDAH) es uno de los más frecuentes en la clínica psiquiátrica infantojuvenil. Aunque el tratamiento de primera elección son los estimulantes, existe la alternativa de la atomoxetina, aunque sin criterios definidos de cuándo pueden tener mejor respuesta a este tipo de tratamiento. Respecto a las alteraciones neurofisiológicas y anatomofuncionales, el cerebelo va tomando protagonismo a la hora de comprender este trastorno. Su papel, más relacionado con las vías noradrenérgicas, puede tener una posible implicación en la elección de la atomoxetina. Así, se presenta un caso con mejor respuesta frente a atomoxetina que a metilfenidato y alteración en la neuroimagen en cerebelo.
